Parents who plan to enroll their child in Sumter School District for the first time are encouraged to enroll as soon as possible to avoid last minute crowds. Parents may also register their child in the school zoned for their residence during regular school hours. Persons unsure of their school zone may contact the district’s attendance office at 803-469-6900, extension 507.
Children must be four or five on or before September 1 to enroll in preschool or kindergarten, respectively. State law mandates that children ages five to seventeen must be in school. Parents who choose not to send their child to a public or private school must sign a waiver to that effect. Waiver forms are available at the district administrative office. Parents who plan to enroll their child in any grade are asked to do so as soon as possible. School offices are open year round for registration.
Upon registration, parents are required to provide school personnel with the child’s official birth certificate, proof of residence, and immunization records. Parents of children transferring from another school district are encouraged to bring the child’s latest report card.
Students new to Sumter School District must complete SC Immunization Form DHEC 4024. A child cannot be admitted to any school or child development program without a valid SC Certificate of Immunization or a medical, religious, or special exemption. For details, contact the Health Department or your physician. According to state law, if your child is transferring from an out-of-state school, his/her health certificate information must be on a South Carolina form. Out-of-state certificates may be taken to a doctor or clinic in South Carolina to inquire what previous information can be transferred to the South Carolina form.

Medical Homebound Instruction
Pursuant to South Carolina’s Regulation 43-241, Medical Homebound Instruction is available for students who cannot attend school, regardless of any or all accommodations provided, due to accident, illness, or pregnancy. Medical Homebound Instruction is intended to provide academic assistance for students experiencing a medical crisis until the student is able to return to school. This service is appropriate for short term intervention and should not be viewed as a long-term replacement for regular school attendance. The goal is to help the student successfully return to school as soon as possible
Step 1: A parent/guardian may make a request for their student to receive Medical Homebound Instruction. The Medical Homebound Instruction application must be completed by a medical practitioner.
Step 2: The parent/guardian must have the student’s medical practitioner read and sign the Physician Letter.
Step 3: The student’s completed Medical Homebound Instruction application, along with the signed Physician Letter, can be email, mail, fax or delivered to Sumter School District, C/O Kathy Strickland, 1345 Wilson Hall Road, Sumter, SC 29150 or Kathy.Strickland@sumterschools.net. Fax number is (803) 469-4006.
